The Pediatric Nurse Practitioner with experience in pediatric cardiology is a strong candidate to join the cardiac transplant team at this leading academic children's hospital as the PNP Heart Transplant Coordinator. The Transplant Coordinator Nurse Practitioner has responsibility for all phases of the transplant process from initial referral through outpatient follow-up. This is a full time, days, 8 hour shift position with rotational call as required for donor/recipient transplant coordination and working with organ procurement agencies.

Responsibilities

Responsibilities of the PNP Cardiac Transplant Coordinator Job:
Assess heart and lung transplant candidates and recipients by obtaining histories and performing physicals
Order, perform and interpret laboratory and diagnostic tests and procedures
Formulate, implement, monitor, evaluate, and revise plans of care that include interventions and treatments to attain expected outcomes
Serve as a liaison between patients, families, medical staff, referring physicians and insurance companies
Assess needs, develop, and implement educational plans and support for transplant patients and families
Participate in the outpatient transplant clinic
Assess appropriate nursing interventions relating to effective use of resources and quality assurance
Demonstrate knowledge, understanding, and compliance with the Organ Procurement and Transplant Network and CMS regulations
Participate in quality assurance and performance improvement meetings and initiatives
Assume rotational on-call duties for coordination with donor/recipient transplants and organ procurement agencies
Develop clinical protocols, policy, and procedures
Provide community education and promote and maintain positive public relations

Qualifications

Qualifications for the PNP Cardiac Transplant Coordinator Job:
Current RN license in California
Basic Life Support (BLS) certification
Pediatric Advanced Life Support Provider (PALS) certification

Master's degree in Nursing
