90501 - This is a great opportunity for the Acute Care Nurse Practitioner with cardiac transplant and/or mechanical circulatory support experience to join the team at the Heart and Vascular Institute of this leading academic medical center as the Nurse Practitioner in Heart Failure and Transplant. The Nurse Practitioner job in this cardiac unit will offer the opportunity to care for patients ranging from medical management of those with heart failure who will undergo transplant to patients with Mechanical Circulatory Support (MCS) as a bridge to transplant or as long term support.

Responsibilities for the Acute Care Nurse Practitioner Job:
Manage new referrals for Advanced Heart Failure (AHF) therapies in both inpatient and outpatient settings
Obtain patient and family histories, perform physical examinations, and assess patient conditions
Order diagnostic testing
Oversee hemodynamic inpatient care ensuring optimization and adherence to quality standards
Prepare heart failure patient summaries for presentation to the AHF Multidisciplinary Committee
Provide outpatient medical management for AHF patients including routine followup and acute situations
Review outcome data and analyze quality metrics
Develop, coordinate, and implement quality improvement plans
Provide support and advanced education to staff, patients and families regarding cardiac transplant and VADs
Qualifications for the Acute Care Nurse Practitioner Job:
Certification as an Acute Care CRNP in Pennsylvania
Clinical Transplant Coordinator certification, or must obtain within 18 months of hire
ACLS and BLS certifications
